Basically the title. The first time I tried last year, my doctors office said they’d file it for me, and we paid $200 for them to do it (I’m very messed up so it took like- 2 weeks for them to finish it 😭). Well I called ACR or AISH (I can’t remember sorry) about 6 months later cuz I hadn’t heard back and they said I didn’t have an application with them, and I had to get a second set of my medical documents because I got the og done in 2025 but I needed them to say 2026. So my doctor gave me a printed copy of the long ass medical documents and history for me, which was Part B, but I can’t find an accessible copy of part A that I can print off because I need to be able to hand it in physically. If anyone can help me find the part A, or even share it with me, that would be a lot of help.
