I am genuinely asking where does the money come from? My suspicion is your taxes are such that you are not net beneficiaries so that your programs are solvent. However, at least in the country that I am from, we have state healthcare for those with financial difficulties and everyone else has private insurance. So no, people don’t die in that manner.
