I would like to change jobs, but I might be stuck. Would love any input

Wait 5 sec.

I have worked in the ICU at a critical access hospital for almost 10 years. We are a mixed unit, so we can take ICU level, and also PCU step down type pts, even MedSurg. I absolutely love having ICU patients. Of course being critical access limits what we can handle, but I can monitor an intubated pt on multiple pressors, understand how it works, and keep them alive (usually). I have had my CCRN for years and try to stay updated on critical care with online classes. Recently I have been looking for a new job in a bigger ICU, but it’s not going well. I have only had a couple of interviews and I was not experienced enough to hire (no CRRT, for example). There are ICU fellowships opening in the spring that one interviewer recommended to me. So here’s my situation. Too old for residency, too inexperienced for real ICU, too restless for non-critical. The fellowship would be perfect. Do you think I would do better as an internal candidate? I could apply for a PCU position at the hospital, and would more likely get hired, then could apply for the fellowship. I took a pretty hard ego hit with the interviews I had so I quit applying anywhere else, but there are quite a few options around me, and I don’t mind driving for work- should I just keep applying different ICUs? Any insight on this? My current job isn’t exactly bad, it’s just not as challenging as I would like it to be, and the critical patients are not constant. It wouldn’t be horrible to stay, but I’m ready for something new. I know the decision comes down to me and my life, but I would love any insight.
