first ever medication error in my 8 months as a nurse

Wait 5 sec.

in my pedia (4mos) pt.. the medication that was started waa clindamycin (dalacin c) 53mg to give every 6 hours. i started it last wednesday. available was 150mg/ml, 4ml. (600 in total) so dosage would be around 0.35 mg i made the mistake of using the 150mg in the calculation and i got 1.41 instead. i did got it first but i made rhe mistake of getting second opinion using AI... who misunderstood my question.. i know i shouldve cross computed with another nurse but i was so under the impression that it was correct.. i was just made aware this day by our head nurse, i feel so bad what if i made the patient worse. i wrote an IR. and the head nurse made the pedia doctor aware. im so scared, making a report is one thing, but im scared of the effects it will have on the baby. how do i cope with this? i feel so mortified and scared that i might cause the pt harm, and what will become of me? please give me advice submitted by /u/y_iraeubri [link] [comments]
