I feel like this is facility dependent, but I’m wondering if anyone’s hospital has this avoidance in ordering foley catheters on patients? I am a surgical nurse working on an ortho surgery floor (with medicine overflow). I worked for 3 years on another similar unit at a different hospital, and just recently switched to another facility. I’m nightshift. The one gripe about my new job is that they literally will NOT order a foley. I have had numerous situations where I have gotten patients that are in the hospital for days and just constantly being straight catheterized. One patient had 11 intermittent catheterizations and then most recently my patient the other night had 16. I try to do my best overnight to ask hospitalist if they want foley and explain how the patient has had numerous intermittent catheters, but they always say refer to day team and then day team never wants to order one. It’s rare I see one at all here. I was told there is this fear of getting dinged for CAUTIs, but I just feel this is causing more suffering and poses an even bigger risk of infection for patients. In fact, my last patient who needed it 16 times, I was unable to get urine on my last attempt and even had to try with a coude with no luck. Had to notify medicine team and get uro involved. Probably had so much trauma from all the catheters (plus BPH of course). My last hospital had a pathway or protocol that after 2-3 times we reach out and they order a foley.
