I've been having one sided pelvic pain for a while, not long after I had ovarian torsion last year and had to have emergency surgery. I kept thinking that there must have been some of the dermoid cyst left behind and an MRI confirmed it. However, my gynecologist said that the fatty/hemorrhagic remnants of the cyst wouldn't cause pain. She seems really adamant about it. She said based on my symptoms I could have adhesions or endo and offered a lap to see. She said if there was any endo she could burn it. My worry is that that sounds like ablation not excision which if it is endo I would need something more radical than simply burning it off, because it could recur and I'd rather not continue to have to need future laps. But this gyno has this way about her that makes me really really really nervous to advocate for myself because she talks over me a lot, or talks down to me or gets annoyed when I ask her to explain something. I felt extremely uncomfortable asking her whether ablation was the best option and if excision would be better because she has the kind of attitude of thinking patients know too much or whatever. She said she couldn't guarantee removing the cyst remnants from my ovary and that spillage is v common and doesn't cause pain. But I can't reconcile that with the reality of my symptoms which are one sided (on the side of the previous torsed ovary and tube that the surgeon wrote was dark red at the time when she was doing the lap on me last year). If it's not the remnants I think it could be adhesions but then am I getting stuck in a cycle of surgery, adhesions, endlessly? Would the fat on my ovary make it worse then? I've actually asked for a second opinion because my gynecologist told me there's a 50% chance she doesn't find anything which is just insane to me if that is the case. The amount of pain I'm in, I just refuse to accept that there's not a clear cause for it and it's so frustrating because I went through the same thing last year - gyno said to me that my ultrasounds were all clean, to go on the pill to relieve my symptoms and I had to fight to get the MRI that showed I had a dermoid that caused torsion. My current gynecologist is a urogynecologist because I have urinary symptoms too, so I just don't really like the idea of her operating on me when she's not an endo specialist and isn't willing to refer me to one either - she said without imaging such a referral would be rejected which puts me in such a catch 22 situation. Previous laps have not been by endo specialists either, though I recognize that the most recent one I had was an emergency so endo probs wasn't the first thing on their minds. I do have a lot of the hallmarks of endo like dyspareunia, painful bowel movements, alternating diarrhea and constipation, insanely painful periods and pain throughout my cycle that seems concentrated on one side. But my gyno has just been flip flopping and changing her tune a lot. First she said I should try birth control and when I said I didn't want to (I was on the pill previously and it didn't help, and IUDs or coils are non options when I can't even tolerate a smear test) she said that if I did have endo it was so "mild" that it couldn't be seen during my last lap. What I want is to request seeing someone else (which thankfully she agreed to) and ideally I want that other consultant who I've seen previously and who is nicer, to consider whether I can be referred to a specialist or at least have her review the imaging and lap pictures. I've not seen any of the lap images myself and I did request them but have not been provided them. Idk if they took photos in an emergency setting tbh. Does anyone have suggestions about how to best broach this conversation with the other gyno I will be seeing? How much worse is ablation than excision? Is it better to get ablation from a non specialist than excision from a non specialist? Bear in mind the other gyno I've been referred to isn't an endo specialist but seemed more open minded when I saw her last year so I'm hoping I can be open about how the symptoms are affecting me.
