I have found that each time that I connect my bluetooth audio devices to the phone the volume resets to the loudest setting. The change seems to happen as soon as I start audio the first time after connecting. When I adjust the volume I can see momentarily that it was on a lower setting and then it jumps all the way up to maximum. This has happened with three different brands of headsets.
